Recurrent Miscarriages Explained

Recurrent miscarriage is a term used by the American Society of Reproductive Medicine (ASRM) to describe someone who has experienced two or more consecutive pregnancy losses, ending involuntarily before 20 weeks. This condition affects about one percent of couples trying to start and family and is used to describe clinical pregnancies only, not biochemical pregnancies.

  • Clinical Pregnancy: Identified via an ultrasound as early as five to six weeks. 
  • Biochemical Pregnancy: Identified via blood or uterine testing.

What Causes Recurrent Miscarriages?

The cause of your miscarriage can be related to:

  • Anatomical conditions caused by a weak cervix or uterine abnormality
  • Chromosomal or genetic disorders caused by either the egg or sperm
  • Endocrine disorders due to hypothyroidism, polycystic ovary syndrome (PCOS), diabetes, or hormone imbalance
  • Advanced maternal age
  • Certain lifestyle factors
  • Maternal immune tolerance, such as antiphospholipid syndrome, thyroid antibodies, increased uterine natural killer (NK) cells
  • Endometriosis
  • Infection

While some cases of recurrent miscarriage are never solved, testing can guide us in the proper direction for treatment.

What to Expect during Testing

image of ultrasoundDuring the testing and diagnostic process, Dr. Kristiansen can perform a complete examination of the uterus using ultrasound technology. This will help her determine if there are any anatomical conditions contributing to the issue. 

Once a miscarriage occurs, it is important to undergo testing to determine the cause. 

If at least two miscarriages have occurred, she may recommend karyotype analysis of both the male and female patient. This advanced testing can identify if there are any chromosomal or genetic disorders that may be causing miscarriages. We can also take blood samples to identify any endocrine disorders or hormone imbalances.

Treatment Options

Once we have completed all tests and have a better understanding of the cause, we can recommend the best treatment for your needs. For hormone imbalances, we may recommend hormone therapy. For patients with genetic or chromosomal disorders, we may recommend in vitro fertilization (IVF) in combination with pre-implantation genetic testing (PGS). Using PGS, our embryologist can locate the healthiest embryos before they are transferred to the uterus.

If you have PCOS, endometriosis, or uterine fibroids, we may recommend medication or a surgical procedure to address the condition and provide a healthy environment for the embryo.
